Completing the visa application form ensures you have the necessary documentation for a smooth entry into your destination. By completing this form, you agree to allow the Suwandi Foundation’s contracted agency, PT Chivesh Benoite Atharrazka, to process your Indonesian C6B Visa. The cost of this 60-day visa is included in your trip payment. If you plan to stay in Indonesia beyond this time, you will be responsible to cover visa extension fees. Once processed, your visa will be emailed to you and you are required to print it out and show it to authorities during your travel. Your cooperation here ensures a seamless journey ahead.

Colored photo or scan must be taken with passport open and laying flat on a solid background. Ensure that no corner is cropped out or obstructed by fingers – be especially aware of the numeric sequence at the bottom of the ID page. Also ensure that there is not a light reflection or glare that makes the page difficult to read. Your top signature page must be signed!

Background Check / Reference Letters

The Suwandi Foundation requires all participants to present a valid background check generated within 1 calendar year, which can be obtained from local police departments or online at sites such as sentrylink.com or beenverified.com. In place of a background check, we accept 2 character reference letters. These should be from two different people such as an employer, colleague, or professor (this cannot be a relative) and must include how the reference knows you as well as their contact details and signature.

Medical Insurance

It is required that all participants have international medical insurance with coverage in Indonesia. In the event of injury or illness, we will always take care of participants’ well-being. But, participants and their families are responsible for medical expenses incurred. Here you are required to upload proof of insurance including the insurance policy number.
